Our annual Awards Night is May 24th at 6pm.  If your student is receiving an award that evening, you should have received an invitation in the mail for it.  It is a bright yellow envelope from the school. Please plan on attending!

Awards given will be from the following categories:

Teacher Awards, Aloha Award, Principal’s Awards, Heart of Gold Award, Peer Tutors, Student Body Officers, Janis Cristofani Award, 8th Grade CJSF medallions, and district competition winners (Spelling Bee and Geography Bee, Odyssey of the Mind, Honor Bands, Choir & Music).

A list of students invited is also posted on the window of Room 8.  If you have any questions, please email Heather Tittle at

Medication Pick Up

With the end of the school year fast-approaching, our records show that your child had medications on campus during this school year.  Please plan to have an adult pick up any unused medications by the last day of school, June 1, 2018.  I do not have any way of appropriately discarding medications, so please come and get them so that they are not taking up space in my medication cabinet at the beginning of the next school year when new medications are being brought in.

At the start of each school year, a new Medication Authorization Form needs to be filled out and signed by you and your physician.  Please find this on the website under the parents section.  If your child has a life-threatening allergy for which Benadryl/Epipen may be required, use the HCP Life Threatening Allergies form.  If your child has other medications, you may use the Medication at School form.  If your child’s physician wants your child to be able to carry an inhaler medication, the bottom part of the form requires a second signature by you and the physician.  Please note that any and all medications, even over the counter medications such as Tylenol, Advil, cough drops, seasonal allergy medications, etc must be accompanied by a physician form.  This is a SRVUSD  policy.

If you have any questions, please call the school office for clarification or feel free to contact me via email (

7th Grade Students 18-19 TDAP Needed

For the 2018-2019 school year all students entering, advancing or transferring into 7th grade will need proof of an adolescent whooping cough booster immunization (called “Tdap”) for school in the fall.  Every doctor’s office has a simple form ready to complete and can be faxed directly to the LCMS office at 925-837-3512.  This requirement applies to all public and private schools. The new requirement was signed into law in 2010.

Please remember that Personal Belief Exemptions are no longer accepted by the school and/or district.  All students entering 7th grade who had a personal belief exemption on file in elementary school will need to comply with obtaining all immunizations unless there is a medical exemption per your MD that is documented with a letter turned into the school office before the 1st day of school, August 13, 2018.  Students who are non-compliant with this policy will NOT be able to attend school until immunizations are completed or a medical exemption letter is obtained from the physician and submitted to the school.

The Tdap requirement affects all incoming 7th grade students in California, so it’s important to avoid the back-to-school rush and to get children vaccinated now. Tell other parents with children in the 6th grade about the requirement and encourage them to schedule the shot early, before summer. If your child has already received this vaccination, please send in the documentation NOW and do not wait until summer or the start of the school year.  We are requesting all documentation to be turned in BEFORE August 10, 2018 so that there is ample time to input all the information.  You may also scan the document and send it to either Victoria Felix or to Anju Lashkari, RN School Nurse at LCMS at their email addresses given below.

The California Department of Public Health and the California Department of Education have been working collaboratively on this important measure. Information on the new Tdap requirement, including information on where to get free or low cost vaccines, forms, and FAQs are available at

Please contact Victoria Felix with any questions at  (925-855-6800) or Anju Lashkari, RN, C-PNP at

Thank you for your attention in this very important matter.
